Simply ‘Tox Club Membership

Terms and Conditions

Our Simply ‘Tox time initiation fee of $59 as well as your first month and last month will be charged at the time of membership set up.

1. Your Simply ‘Tox membership agreement begins the day you sign up. Your monthly membership payment will be automatically deducted on the same day you joined each month using the debit card or credit card provided upon sign up. Botox treatments are to be redeemed every 4 months up to 3 full treatments in a 12 month period. Treatments are non transferable and can only be used by stated member.
2. If you request additional units in between appointments, you can purchase them at the discounted member price of $11 /unit.
3. You may cancel your membership at any time after the first 12 month period. Cancellation must be done before your 1st treatment in the 13 month of your membership. Cancellations are to be made via email and sent to info@simply­medspa.com. We will contact you to confirm your account cancellation within 48 hrs.
4. You may place your account on hold once a year after the initial 12 month period. Hold requests are to be made via email and sent to info@simply-medspa.com by the 1st of the month for the following month.
5. lf your monthly payment is declined, you will be given 7 days to reconcile your account. If not reconciled, we will charge you for all previous Botox units used at the full price of $15/unit.

This agreement gives SIMPLY.AESTHETICS MEDSPA the authorization to charge your credit or debit card for monthly membership dues. We reserve the right to modify, discontinue, or change services and processes without any notice to ensure the maximum standards of quality care and services are being met. Simply. Aesthetics reserves the right to discontinue a service or treatment for any reason.
